So to be clear you want to use two times 5x10x4 bearings with 2mm between bearings , offset 4.5mm outward so the drive cup sits properly in the hub/knuckle .
If a jobs not worth doing then its certainly not worth doing well.

To save any confusion don't think about the bearing sizes. Just make the knuckle extender 4mm thick thanks.
I'll tinker with the bearings.

Ok so just a 4mm extender with larger holes for screws to slide through .

I've printed a couple of the ones you wanted today , I found the Zerda bearings I have are 5mm thick so I made a 5mm thick version as well , I'm printing one of the version for the Hotshot axle at the moment , in my mind the one for the Hotshot axle is a better bet as both bearings sit in it where the hub extender version has one in the extender and one in the stock hub and only the two screws align them , I just feel there's a good chance that the extender could be knocked out of alignment , I was fully planning on sending a set of 4mm and 5mm hub extenders as well as a set of the Hotshot variety anyway .

4mm extenders should be fine, 1mm of the 5mm bearing will sit in the stock hub. i'm confined to the axles I've bought and the new 4mm wide bearings plus the zerda 5mm bearings so the two bearings only extend by 4mm. My other option is use two 4mm thick bearings with a small 5x10x4 inside it making total thickness 8mm and I'd put a spacer washer 1mm between them if needed so total 9mm needing 4mm extender in either case.
If I use 5mm thick bearing 1mm would sit in the existing hub and 4mm in the extender so shouldn't budge and could put a bit of thread lock or double sided tape between if needed

It looked to me that your hotshot axles take two 5x10x5 bearings or similar?

The genuine ones I have are old and a little worn so these will get an old gal rollin again .
