That's a nice variation on the PanaVise 366 head.
If the shaft is 5/8" then it will slot right into a 300 series base.

Are you referring to Gold Top Loaders or Bottom Loaders? O-rings are easy, so many sources. I have a chassis laying around that looks just like yours without the milling. I've always considered it a piece of junk, but your chassis drill holes are damn near identical. Weird.
